REFERRAL

These are many ways that you can access services at DASSI.

You do not need a referral from a General Practitioner. You can access DASSI services directly or they can be arranged by your care coordinator as detailed below.

• Self Referral (Yourself, family member or carer)

• Allied Health Professionals and General Practitioners

• Community Health Services

• Disability Service providers

• Case Managers and Coordinators (Transport Accident Commission, Workcare)

• Department of Human Services
